Addressing Community Health Needs
Every three years, Stanford Health Care, in partnership with local public health departments, community health experts, and residents, conducts a robust assessment, discovering ways to better serve our most vulnerable and underserved communities.

The 2022 Community Health Needs Assessment (CHNA) identifies pressing health disparities, uncovers gaps in care, and offers strategies to address the drivers of and barriers to poor health across Santa Clara and San Mateo counties. These are the factors that patients and their families bring with them when seeking care at Stanford Health Care and influence their ability to get healthy, stay healthy, and be healthy at home.
Based on community priority and severity of disparities, between 2023 and 2025, Stanford Health Care will address the following community health needs (click the icons to view):
