Health Needs Addressed: Access & Delivery to Health Care, Behavioral Health

This program provides health care and supportive care services to address the “total health” needs of homeless patients post-hospital discharge, including intensive case management, behavioral health (mental health and substance abuse) services, and linkage to community-based social services.
