Best Practices
Hospitals that employ a best practices approach to care demonstrate an ability to coordinate complex care processes. Care coordination is especially important when caring for heart attack, heart failure, pneumonia or surgery. Best practices, also referred to as Core Measures in the hospital setting, describe the key steps hospital staff should take to provide appropriate care and minimize complications. Stanford Health Care is a leader in offering coordinated complex care and has recently implemented an additional improvement program.
Sepsis program
Sepsis is the body's response to infection — an inflammatory process marked by an elevated heart rate, rapid breathing and abnormal temperature. Even a minor infection can trigger sepsis, and hospital and surgery patients are at an increased risk. It's usually not life-threatening, but complications of sepsis can cause serious illness and death. Stanford Health Care recently launched a major performance improvement project focusing on early detection and treatment of sepsis that includes hospital-wide education on recognizing early signs and symptoms of sepsis as well as ongoing monitoring and evaluation to improve early detection and treatment.
