How the Program Works
At Stanford you will be seen by a health care professional who specializes in the type of cancer you have. After you complete your treatment, your team will help you develop a plan for follow-up care that will include survivorship visits to help you lead a healthy and active life after cancer.
We recognize that recovering from cancer can be a challenging process. In addition to conducting regular monitoring during these meetings, we will be here to answer any questions or concerns that are coming up for you. We can also offer ideas for lifestyle choices that you may find beneficial. Our role is to serve as a resource as you heal.
Your survivorship visits will include:
A review of your medical history
Evaluating and finding solutions for treatment-related issues
Offering tips for healthy living habits like diet and exercise
Addressing concerns about anxiety, depression, relationships, or returning to work
Referrals to outside experts and resources
A Survivorship Care Plan document for you and your primary care physician (PCP)
It is important to see your primary care provider for regular visits. He or she will be routinely updated on your follow-up plan of care.

Please fax the Medical Record Release Form to your new patient coordinator. The medical release form is an authorization form for external facilities to release medical records to Stanford Health Care.
