For Patients

PREPARE FOR YOUR APPOINTMENT

We usually obtain notes from your doctor prior to your visit, but encourage you to bring any medical documentation available to you. Please also bring the following:

  • Your medication list and the actual pill bottles for those medications
  • Test results including laboratory tests; please obtain the actual images (on a disc) for echocardiograms, angiograms, and stress tests
  • If you do not have some of this information and need to have any diagnostic testing performed, our clinic will help you set up appointments.

DURING YOUR APPOINTMENT

  • You will meet our heart failure team. We will review your history and records. You will learn about your condition, receive recommendations about dietary changes, and have medications adjusted.
  • Tests may include an echocardiogram, an EKG, defibrillator/pacemaker testing (if applicable), and blood draws to test kidney and liver function.
  • Your visit will typically last one hour or longer.
