Putting Patients First—Including Yours
At Stanford Health Care, we welcome the opportunity to work with you and your patients. Our referral process is simple and you can contact a physician liasion with any questions regarding access to our online portal, follow-ups, in-person meetings with our physicians, community events and more.
1. Directly via Epic using CareEverywhere Referrals Management (CERM)
If your organization uses Epic as your Electric Health Record (EHR) system and already has CareEverywhere Referrals Management (CERM) enabled, you can refer to our Places of Service as you normally would. If associated with the correct Direct Address, your referral will be processed through the CERM workflow.
As of February 2023, we are on Open CERM. CERM is our preferred method for all Consult, Test & Treat referral types from other Epic systems.
Note: We do not accept Radiology Orders or Procedure Orders through CERM. Please use Prism or fax, instead.

2. Online With PRISM
Through our online patient referral portal, you’ll be able to access updates on your patients anytime. Just log in, enter your referral information, and attach any relevant records. If you’re not sure which Department or Department Specialty to select, choose “Provider Relations” or call our Prism Helpline at 650-724-1314.

For prompt processing, please download our Stanford Health Care Referral Form and fax it to 1-650-320-9443.
Send your Referral Form to the Referral Center at ReferralCenter@stanfordhealthcare.org.
For prompt processing, please make sure to fill out the following sections:
- Patient's name
- Date of birth
- Patient address
- Patient phone number
- Clinic requested/Stanford provider of preference
Please note: Documents that are in any format other than a PDF will not be processed. Encrypted messages, external accounts to login, or links, will not be accessed.

Transferring a Patient
Our Transfer Center nurses are available to assist you with transferring patients to Stanford Hospital or Stanford Health Care Tri-Valley. Call 1-800-800-1551 or learn more.
For Referring Physicians
We are here to ensure that your referring experience is a positive one. Our nurses are happy to facilitate and monitor requests for you and your office staff.
Call our Physician Helpline at 1-866-742-4811 to speak to a referral representative who can help you:
- Connect with one of our physicians by phone
- Stay up to date on your patients’ referral progress
- Navigate referrals to the appropriate clinic or specialist
After hours: Please call the hospital page operator at 650-723-4000 and ask for the physician on call.
