Care and Treatment at Stanford
Treatments
The Oral Anticoagulation Clinic (OAC) of Stanford Health Care offers a comprehensive range of patient-focused pharmacy services. The goal of OAC is to optimize the patient's anticoagulation therapy with warfarin and/or low-molecular weight heparin.
Patients are carefully monitored and followed using an established anticoagulation protocol including lab draws and follow-up visits or phone calls. In addition, the clinical pharmacist is available 24 hours a day via pager for emergency situations regarding the patient's anticoagulation therapy.
The clinic is currently managing 30-40 patients per day and receives referrals from multiple services including Cardiology, Oncology, Transplant Surgery, Hematology, Primary care/Internal medicine groups.
Oral Anticoagulation Clinic is a critical source of patient support and education since the staff interfaces with each patient at least once a month. The staff also provides educational services about anticoagulation, possible interactions, possible complications and side effects.
Anticoagulants
Also known as "blood thinners," anticoagulants are a type of medication used to prevent or break up clots in blood vessels or the heart.
For Patients
All patients must be referred by a Stanford doctor.
For blood draw only, contact us for a requisition order good for six months. We request that routine blood draws be done for patients who cannot be seen during the regular Oral Anticoagulation Clinic appointments.
